RESUMO

During the fifties and sixties, physicians were mostly prepared as general practitioners. However, the progress in scientific knowledge and the introduction of complex technologies required the training of specialists. A number of sub specialties have been progressively added to the four basic specialties, namely Internal Medicine, Pediatrics, Obstetrics and Gynecology and General Surgery. The Post Graduate Schools of the traditional universities devised courses and training periods to teach these sub specialties. The Association of Medicine Schools has been in charge of the accreditation of the new training centers. Since the training of new specialists required the collaboration of hospitals of the public National Health Service, there is a special commission that coordinates the relationships between teaching requirements and medical attention in these hospitals. There is still a controversy on the exact proportion of general practitioners and specialists that the country needs, the role of Family Practice and the role of prevention and health promotion

RESUMO

Sustaining quality control of learning programs for health professions has become a central issue in university systems, under the pressure of an unexpected merging of new medical schools in Chile, during the last decade, and the massive arrival of other Latin American physicians. Accreditation of institutions and programs represents valid safeguards used in most countries where professional training processes take place. Due guarantee of its quality is required to assure proper health care to people. The authors provide specific arguments to support systematic evaluation and accreditation processes, as those introduced in our country to fulfill the requirements of a high level medical practice

La autoevaluación y la autorregulación sirven para determinar el funcionamiento y los logros del programa, que se revierten en la capacidad de retroalimentar su funcionamiento y procurar su excelencia, cuyo fin es demostrar que el programa que se somete a la acreditación tiene calidad, la cual es verificada por pares externos a la institución o programa.El Programa de Enfermería de la Universidad de La Sabana, desde el inicio de sus actividades, ha realizado autoevaluaciones que le han servido para mejorar sus procesos y retroalimentar sus acciones en la formación de estudiantes y profesores. No ajena a lo determinado por la Ley, se inscribió en el proceso de acreditación, y para tal fin, siguiendo los lineamientos dados por la Dirección de Acreditación y Currículo, realizó una autoevaluación del programa en los diez factores propuestos por el CNA; analizó cada una de las 66 características y las 200 variables, y emitió juicios basados en documentos de análisis y soporte y en encuestas de opinión aplicadas a estudiantes, profesores, directivos y egresados, acerca de la realidad académica y administrativa, lo cual permitió determinar las fortalezas y debilidades y proponer planes de mejoramiento en el fortalecimiento del programa

RESUMO

The Medical School of the University of Chile is enforcing deep transformations in its curricular setting, to cope with the physician model of the next century. It is following the universal trends that look for new objectives, methodologies, scenarios and resources in medical teaching.These goals should be accomplished very soon, allowing the School to be in the lead of curricular transformations. These changes have overflowed the limits of curricular renovation and a process of institutional reorganisation is aiming to increase its efficacy and yield. The bases of this reorganisation are the search for excellency, the renovation of academic staff with the incorporation of better trained professionals, the integration of basic and clinical disciplines in coherent programmatic proposals and the configuration of structures to organise and articulate medical knowledge.Optimisation of education has also extrainstitutional implications with the establishment of accreditation systems for professionals and educational institutions.These systems are mandatory world wide and guarantee the capability of educational institutions and their products. The School of Medicine is committed in this achievement along with the Association of Medical Schools and The Superior Education Council

RESUMO

The National Committee of the Association of Medical Schools for the Accreditation of Medical Centers to train specialists is active since 1979. At the present time, 1547 postgraduate medical students are engaged in training programs lasting 2 to 3 years. During the period in which the Committee has operated, 591 centers were assessed. Sixty percent were approved but the deficient academic or technical facilities for clinical practice or teaching motivated the rejection of 14 percent. During these years, several problems were detected such as the support given by the authorities of medical schools to accretitation, the instability and shortage of teaching faculty, the persistence of training programs in rejected centers, the excessive number of students for the capacity of the teaching center, the faculty integration between Medical Schools and the National Health Service and the increasing competition of scientific societies and private health institutions with medical schools in teaching activities. These issues must be appraised both at medical schools and the National Health Services
